ETA: The HTML totally works on my website, but somehow not on LJ--possibly delete the codes from the beginning and end? LJ might not accept them, for they are actually supposed to represent notes for the sitemaker to remember that won't show up on the site.

Actually it is; it stands for hidden text that only shows up in the source code. *did lengthy and overdramatic explication of it above that totally FAILED AT helping fix the problem*
I'd guess that the problem is caused by using the rich text editor as well ... if you want to post raw HTML, you have to be using the basic editor, or else everything you type in is assumed to be ordinary text, and all the < and > characters are 'escaped' and replaced by HTML entities so it doesn't get interpreted as HTML markup. If you use the 'View Source' command on the page, all of the < and > characters in that block have been replaced by < and > respectively.
Heh, I can't help you at all because I'm crap with computers, but it made me laugh because there's all this html and then in the middle - it caught my eye - it just says "The Weird Sisters".

The problem is, in my very humble opinion, that you had the rich text formatting on when you cut/pasted this ... but if people click on the link at the very end, they can see the survey properly.
