I think there were plotlines in store for Inara that are... disturbing, to say the least, but from what we were given, that wasn't the case, and who knows what actually would have happened had the show continued.

And yes, it's clearly meant to be a Mal issue. Interestingly, since he was once quite religious, I wonder how much of it is also carry-over from that period of his life.
